The disintegration of a B-52 Stratofortress bomber on January 21, 1961, released two Mk.39 Mod. 2 hydrogen bombs, which landed near Goldsboro, North Carolina.

During the fall of one of the bombs, its parachute opened and the triggering mechanisms began to work. A fourth safety mechanism prevented the bomb from exploding on impact; the previous three were disabled (one mechanism prevented an accidental explosion on the ground and the other two during flight).

A secret document about this event was published by the British newspaper The Guardian on 21 September 2013.
