Pakistan operates a total of 4 Il-78MP aircraft. All 4 were acquired in 2006 from Ukraine, where the aircraft remained after the collapse of the USSR as part of the 409th Refueling Regiment under the 106th Heavy Bombardment Division and were stored. Pakistan paid $25.68 million for the aircraft and the aircraft were upgraded to Il-78M standard at the Nikolayev Aircraft Repair Plant and given the Il-78MP index. The upgrade of the first aircraft (R09-001, formerly USSR/UR 76742, serial number 0073478346 and serial number 59-07) was completed in 2009 and it entered service in December 2009. The other aircraft that Pakistan has acquired are registered R10-002 (former USSR/UR-76730), R11-003 (former USSR/UR-76675) and R11-004 (former USSR/UR-76682). The contract included an option for a fifth aircraft, but this was not exercised by Pakistan.
In 2017-2018, one of Pakistan's tankers (R09-001) underwent an overhaul at Russia's 360th Aircraft Repair Plant in Ryazan. The tender to overhaul the second aircraft in 2020 has already been won by the Ukrainian company where the machines were originally upgraded, but Ukrainian sources openly admit that the reason for the win was a dumped price that does not even cover the cost of salaries. In June 2021, a contract was signed for the repair of a third tanker, again with a Ukrainian company.
