Vznikla 1. července 1919 přejmenováním Severní armády. Rozkazem velitele Severozápadního frontu č. 77 ze dne 2. října 1919 v souvislosti s převzetím velení armády gen. Juděničem byl štáb armády přejmenován na štáb Hlavního velitele Severozápadní armády. Armáda byla likvidována v souladu s rozkazem velitele armády č. 57/op ze dne 22. ledna 1920.
