Components: 		Dekoraci tvořila medaile na pětiúhelníkovém kovovém závěsu potaženém stuhou, nošená na levé straně hrudi. -
Klenot:
Badge: 		Medaile je zhotovena ze stříbra a má tvar kruhu o průměru 36 mm. Do středu lícní strany je vsazen reliéf poprsí F. F. Ušakova s tečkovanou obrubou. Po obvodu horní poloviny medaile je rozložen nápis v azbuce „ADMIRAL UŠAKOV", rozdělený uprostřed pěticípou hvězdičkou, dolní část zdobí vavřínové ratolesti spojené stužkou. Medaile spočívá na kotvě, jejíž rameno a hroty přečnívají nahoře a dole okraje kruhu. Na rubové straně je kotva. Stuha je světle modrá, 24 mm široká a po stranách má úzký bílý a tmavomodrý pruh. K medaili je připevněna stříbrným kotevním řetězem. -
