The medal is a somewhat lower award of merit than the 1906 Cross of Merit.
This medal was established by decision of Charles I as early as 8 April 1878. Its purpose was to award for merit and bravery the citizens of Romania, both soldiers and civilians. The medal could also be awarded to foreigners for general merits in the field of culture and science, or for services to the Romanian Kingdom. From its foundation in 1878 until 1932, the medal existed in two classes
1. gold medal in gilt silver
2. silver medal
On 07 April 1932, the Romanian King Charles II ordered the establishment of another grade, the bronze medal. Nevertheless, the medal was awarded very often and can be found on the clasps of the Order.

On February 12, 1937, swords were added to the Meritorious Service Medal for services in the field of war. At the same time, the medal with the swords could be awarded on the so-called war ribbon (Blue ribbon with silver threads on the edges and trimmed with gold edges, thus similar to the ribbon of the Order of Michael the Victorious.)

The medal was often awarded during the years of World War II. In this case the ribbon was supplemented by two gold stripes on the edges of the ribbon.
