Sformovaná bola od januára do februára roku 1943 z 3. záložnej armády (druhého formovania). Na začiatku boli súčasťou armády 11. a 16. tankový zbor, 60., 112. a 194. strelecká divízia, 11. gardová samostatná tanková brigáda, 115. strelecká brigáda, 28. lyžiarska brigáda a ďalšie formácie a jednotky. V polovici februára 1943 bola armáda podriadená Strednému frontu a v mesiacoch február a marec sa zúčastnila útočnej operácie v smere na mesto Brjansk.


V júli a auguste 1943 sa armádne jednotky zapojili do gigantickej tankovej bitky pri Kursku a neskôr aj do Černigovsko-Pripjaťskej operácie. Začiatkom septembra 1943 bola armáda stiahnutá do zálohy Hlavného stanu najvyššieho velenia, v polovici januára 1944 bola začlenená do 1. ukrajinského frontu. V jeho radoch sa koncom januára 1944 podieľala na odrazení protiútokov nepriateľských vojsk na Vinnickom smere, vo februári zase juhozápadne od mesta Korsuň-Ševčenkovskij (Korsuň-Ševčenkovská operácia). V zostave 2. ukrajinského frontu, resp. od polovice júna 1944 1. bieloruského frontu, sa armádne jednotky podieľali na Umansko-Botošanskej a Lublinsko-Brestskej útočnej operácii.


Za vojenské zásluhy bola armáda dňa 20. novembra 1944 premenovaná na 2. gardovú tankovú armádu.
