History of the aircraft:
Shipped on the 4th. June 1965, the first unit 1 PLM, Disaster 16. January 1978, which killed por. pil. inź. Tomasz Smaga.

Progress of the disaster:

It was a training flight around the circuit at night in good weather conditions (about 22:30). A trained pilot Tomasz Smaga, started with the aircraft and after about 2-3 km from the end of the start of the runway, at a height of 200 m and a speed of 600 km/h, he heard the instructor an explosion in the rear of the aircraft, ordered the pilot to turn off the combustion (?), after that there was another, considerably stronger explosion, in addition, started the rear part of the aircraft to "shine", in addition, indicated the warning indicator light the "fire". Instructor this is assessed as a dangerous situation and issued a directive to pilots ejection seat, whereupon 42 with from the start at a distance of 3-3,5 km from the end of the runway, a height of around 250 m, a climb speed of 600 km/h successfully ejected from the cab of no. II (back) landed safely in aerodrome facilities in Mierzecicích. Now stay on the plane in the cabin no And (the front), only a trained pilot, he ejected 21 with after katapultáží instructor at a height of 400-450 m, however, there was no separation of the pilot from the seat and thereby to open the parachute coź resulted in the death of, the aircraft neřízen crashed and was destroyed.
The cause of the accident the aeroplane is not entirely clear, the most likely appears to be bursts of fuel accumulated in the back of the aeroplane because of the leakage of the fuel installation. Cause no separation of the pilot from the seat was that the seat wasn't adjusted as it should be according to the design journal from the year 1971! It was the replacement of the cable interface of the gas connector ORP-2 on no ejection seat, the cable has not been replaced for less. Coź led to the failure of the seat - no separation of the pilot from it and the subsequent opening of the parachute.
