Výrobní (sériové) číslo: 660404, Číslo motoru: G52937079, Zničen při havárii 8.6.1979, Při běžném cvičném letu došlo při nácviku slétanosti ve dvojici chybou vedoucího pilota letícího na tomto letounu ke srážce s vedeným letounem (MiG-21F-13 0405 pilotovaný posluchačem Zdeno Bielikem) a následném pádu obou letounů. Pilot, npor. Jiří Pohán, opustil letoun katapultáží.
